Enhancing Patient Care

UPMC MedTrak

MedTrak is a series of technologies which improve communications and data flow among care providers. MedTrak is committed to providing referring physicians and others a means of improving information flow about their patients. 

MedTrak allows physicians and clinicians to share contact information and communicate with other physicians through a secure network. It acts as an extension of the electronic medical record and provides conditional messaging and anticipatory alerts.  It utilizes Asynchronous Clinical Pathways.  MedTrak supports UPMC MedCall and  Hospital Operators.  MedTrak also provides:

  • Terrorism Response & Information Center
  • Clinical Decision Support
  • Data Quality Initiatives
  • Patient Safety Committee
  • Anti-Black Hole Program

Provider Database:

  •  19,000 physicians
  •  35,000 employees
  •  9,000 other care providers

Contact Information

  • Maintains best available contact information for physicians, providers, employees and others
  • On-Call Schedules listed:  More than 400 on-call schedules are currently online for 10 Hospitals
  • Patient/Physician Relationship Database: Over 755,000 Patients currently listed, comprising more than 1.8 million relationships with physicians. 
    • Patients with a single Primary Care Physician (PCP): 75%
    • Patients with no PCP:  21%
    • Patients with more than 1 PCP: 4%
  • Provides patient perspective: "My Doctors"

Message delivery system

  • Delivers patient related information to hospital and referring physicians and entities    

 

MedTrak can be used for:

  • Any alert (i.e., Delinquency, Missed Appointments, Bioterrorism Alerts, etc.)
  • Upgrading and expanding an existing back end message delivery system
  • Incorporation into the MQ infrastructure to allow easy creation of outgoing message types
  • Monitoring operational processes and providing a "safety net" for failed communication

  • 45,000 Discharge Summaries
  • More than 18,000 Documents/Month
  • Educational materials via Distribution List component (SARS Guidelines)

UPMC MedCall
The mission of UPMC MedCall is to facilitate communication among physicians, institutions, and patients and to provide a single source of "concierge-style" contact for physicians and providers of UPMC and its components.

Answering Service
UPMC Physician Services provides several types of high-quality answering services designed specifically for physicians' offices and other medical groups.
